This order form is intended to be printed and used for reference when placing an order by phone or by mail. It can also be filled out and submitted via e-mail however the order is processed only when a deposit has been received. Please only submit the order form via e-mail if you intend to make a deposit promptly by phone or by mail. We require around 50% down to order and the balance is due prior to completion. We accept all major credit cards, personal checks and money orders. Delivery time varies so please contact us ahead for an estimated completion time on your order. After we receive your order with deposit you will be mailed a confirmation letter and invoice to obtain for your records.
